Per New York State guidelines - the following forms need to be submitted for review by the school Nurse for you to be medically cleared to participate in athletics.

All athletes need to have an updated physical on file in the health office. The physical must be dated within 12 months of the start date of the sport.

Any athlete who will carry/administer their own medication during sports participation is required to have a medication form on file in the health office and the form must be signed by both a physician and a parent and needs to state that the student can self carry and self administer medication. The form is valid for one school year and must be renewed each year.

The Medical Update and Recertification form needs to be completed and signed by a parent/guardian for every sport season a student athlete participates in. The form needs to be completed and handed in to the school nurse within 30 days of the start of the season.

A physician’s note is required to clear an athlete for participation in sports for any injury or illness that occurred since the last physical and that led to the athlete missing more than 5 days of practice.

There is a new APP or Athletic Placement Process for any 7th or 8th grader who wants to participate at the JV or Varsity Level. Please contact the Athletic office with questions.
